Medical Home Southwest Tarrant (MHSW) will serve as a primary care clinic located in the 76123 zip code that will primarily serve residents of the Fort Worth Wedgewood neighborhood in Southwest Tarrant County. MHSW will have behavioral health specialists working alongside primary care physicians in the same space. This will provide outpatient patients with access to counseling, medication, and other medical services to treat behavioral health issues, preventing the need for patients to be hospitalized.
